Find out how your child and family can be referred to Jessie May.
If your child or family are looking to access specialised nursing care or bereavement support from our Jessie May nurses, please ask your GP, Community Paediatrician or Lead Nurse to complete the referral form below.
Or for GP’s and medical professionals; please refer a child or family you support directly below. Further information supporting a referral can be shared via our nhs.net email ubh-tr.JessieMay@nhs.net

Please note: If the referral is urgent, as end of life care support is required – please phone our Care Team on 0117 9582172 after you have made the referral so that we can action urgently.
Once submitted your referral will be reviewed by the Care Team, and an initial acknowledgement will be made within 7 days (via email or phone call)
We will then contact the family to advise of acceptance, and what happens next. Or, in the case that the referral has been declined, the reasons for that.
